Ping Pong-a-thon 9.30-Midnight Consent Form

Week 10 (3/7), during XS, we will be fundraising to support organisations that prevent and support victims of human trafficking by playing table tennis overnight! 

XS Youth will be running the normal time from 6.30-9.30pm + the option for highschoolers to stay later from 9.30pm to midnight. 

Entry for the night is $10 which includes at least 4 slices of pizza. 

This form is for parents/caregivers to give permission for their highschoolers who would like to participate in the Ping Pong-a-thon at XS until midnight.
